After logging in with your cNet or UCHAD ID and password, please complete a brief form providing your contact information and details of your service request. The request will be sent to the Service Center for dispatch. You will receive an email confirming our receipt of the request and providing a service request number that can be used for follow-up. If you have difficulty logging in or placing a service request, contact email@example.com.
